Special educational needs and disabilities applications

This is for transport support for Children and Young People with a Special Educational Need or Disability (SEND) which affects their ability to travel to school or college. For more information about transport to school or college, please see our 2022 to 2023 transport policy.

To apply for assistance with transport to school or college, you'll need to provide documentation to demonstrate the student's Special Educational Need or Disability. These may include:

medical notes
contributions from professionals (we do not need EHCP documents as we have access to these)
photograph (if applying for a bus pass)

You'll be able to upload these documents when you complete the online form.

In addition, there are a few questions which require longer answers. You may want to compose your answers to these questions prior to completing the form. You can then 'copy and paste' your text into this form once you're ready to send it. There is a 500 character limit for each of these questions so please make sure your answers are an appropriate length.

The following questions will be asked:

briefly explain the student's additional needs and why these are a barrier to them travelling independently. We have access to your EHCP (if applicable) so please provide information just relating to travel
please provide details of their medical condition
if the student takes any medication, briefly describe what they take
if necessary, describe any other additional seating requirements
if applicable, describe any other equipment required
how does the student travel when not at school or going to an educational setting?
please describe if the student might get upset or display challenging behaviour - how can this be avoided? What might happen? How might staff best help in this situation?
please briefly describe what will help the student to become more able to relax and enjoy the trip in the vehicle?
