Notification of your event
You need to notify us of your event. You can do this via an online form, before you notify us, please have the following information as you will need it during the form.
- Details of the person responsible for health and Safety
- an event/safety/operations plan
- a Site plan
- a general risk assessment
- a Fire risk assessment
- a Noise risk assessment
- a Traffic Management plan
- a Medical Plan
- copies of relevant public liability Insurance
You will also need to complete a Medical Assurance Form, and return it to the Yorkshire Ambulance Service at least 60 days before the date of your event.
