Use this form to notify the City of York Safety Advisory Group (SAG) that you're holding an event in York.

Before you begin, you must ;gather required information and documents, including:

• details of the person responsible for health and Safety
• an event, safety and operations plan
• a site plan
• a general risk assessment
• a fire risk assessment
• a noise risk assessment
• a traffic management plan
• a medical plan
• copies of relevant public liability Insurance
