Support with physical disabilities and health conditions

We offer a range of services for people with a physical disability or serious health condition (and their carers), including:

  • an occupational therapy assessment to identify how your disability impacts your everyday life
  • advice and information
  • equipment to enable you to remain independent at home
  • adapting your home environment to make it more suitable for your needs
  • offering housing advice to enable you to move to a more accessible property
