Child chaperone application form

If you have not previously been employed as a chaperone see our guide to child performance licences and our guide for chaperones.

You'll need to complete this form if you wish to apply for a Chaperone Licence.

By registering as a chaperone you are acting as a responsible adult for all children in your care, including responsibility for:

• performance times
• health and safety
• safeguarding and child protection

You will also be required to complete a Disclosure and Barring Service (DBS) application. Once completed, the DBS disclosure will be posted directly to you and you will be required to bring the original into West Offices before we can issue a licence.

You will need to send your DBS application payment separately by bank transfer (BACS) or cheque. Payment details will be provided on submission of your application.

A DBS check costs £55.50 if you will be paid for your chaperone duties, or £6.00 if this is a voluntary role.

You’ll need to provide the following as part of your application:

• details and certification for any first aid training completed
• details and certification for any child protection training completed
• details of 2 people who can provide references for you; at least 1 should have known you in a professional capacity
• a scanned copy of a passport style photograph
