Our valued adult social workers support adults, families and informal carers to live an enjoyable life as independently as possible.
We focus on the positives of a person’s life and the neighbourhood in which they live, recognising the capacity, skills, knowledge and potential that individuals and communities possess.
We’re looking for dynamic, motivated, committed and passionate social workers with knowledge and experience of working with adults.
We want social workers with excellent communication and problem-solving skills, who can build trusting relationships with people, colleagues and partners and can work as part of a hard working team of dedicated staff.
We are building back from the pandemic and are looking at how we can improve our practice and develop our neighbourhood model. It’s an exciting time to join us!

Intensive support service
This fast-paced team forms part of a system-wide ‘home first approach’ alongside reablement, community response and intermediate care partners.
Our close-knit team works with people in the community and those who have been recently discharged from hospital. They give short-term, intensive support to reactivate skills, rebuild confidence and ‘reable’ people to live independently.
The community team supports adults from younger adults with disabilities to those in older age.
The work is busy and varied, as the team currently works across the city with a diverse mix of people and with the community.
This community work includes offering residents informal face-to-face conversations at our Talking Points. Here, adult social care staff can talk through options and offer personalised advice.
This is linked to our prevention and early intervention work in communities. We link together services that support people to meet their needs and to remain as independent as possible, for as long as possible.
The team is made up of social workers, social care workers, review managers, customer contact staff, carer support workers and continuing healthcare specialists.
Learning disability and preparation for adulthood team
This team supports young people aged 18 to 25 with a learning disability as they leave education and prepare for adulthood, and also works alongside adults with a learning disability in a strengths-based way.
The approach of supporting people to live positive, active and fulfilling lives is the same regardless of age and situation. The team is made up of social workers and review managers.
Safeguarding and DoLS team
This small team of social workers look at concerns relating to the abuse or neglect of people with care and support needs.
They provide advice and information, conduct necessary enquiries and support people to achieve the outcomes important to them. They ensure that lessons are learnt and that care quality issues are addressed where required. The team works closely with partner organisations including health and the police.
Mental health team
This busy team works alongside health and community organisations to support people, their families and carers with their mental health.
They work in the community to prevent hospital admission. They also work in formal settings to enable a timely pathway to recovery, and a return to the community as quickly as possible.
Occupational therapy team
This team works with children and adults with a perceived need for care and support. They assess the need for equipment or adaptations, and provide information, support and advice.
They also work closely alongside partners such as housing, housing standards and adaptations, falls prevention, GPs and health outreach services.
The work is varied and can involve moving and handling, and major adaptations such as installing level access showers or stair lifts.
The team is made up of occupational therapists and occupational therapy assistants.
Hospital social work team
Working alongside health colleagues, this team supports people to leave hospital and rehabilitation, adopting a ‘home first’ model of care and support.
The team supports people both whilst they are in hospital and throughout the discharge process. This involves working with people who have been discharged from hospital and require further assessment, rehabilitation and support.
The team also has social workers based in accident and emergency and a small specialist team who work with people with ongoing health conditions.
