If you're a young person who's been 'looked after' we can help if you have left, or are about leave care - your social worker can put you in touch with our Pathway Team (see contact details below).
We'll make an assessment of your need from meeting with you, the people who care for you and any other people who know you well.
We look at your:
- practical skills and what you might need help with
- leisure interests
- education, training and/or employment
- need for support
During your needs assessment we write your Pathway plan, which says what you might need help with and who will provide the help.
From then on we look at your Pathway plan together every six months to see what has changed and agree any updates that may be needed.
Your personal advisor will tell you about the groups we run, our newsletters and the wide range of social activities that you can take part in after you have left care.
