Practitioner Registration Application Form

Use this form to apply for the registration of a practitioner, or to amend your registration details, for:

• acupuncture
• cosmetic piercing
• ear piercing
• electrolysis
• semi-permanent skin colour
• tattooing

One application form must be completed by each individual person engaged in the practice of acupuncture, or the business of cosmetic piercing, ear piercing, electrolysis, semi-permanent skin colouring, or tattooing.

Registrations remain valid as long the premises is used for the practice of acupuncture, tattooing, ear piercing, cosmetic piercing, semi-permanent skin colouring or electrolysis, however once registered you must inform us of any changes to the details provided.

The registration is only valid within the City of York Council’s local authority area.

Endorsed photograph application

If applying for a new practitioner’s registration, an endorsed passport-size photograph of the applicant must also be provided in support of your application.

You’ll need to download and provide a separate practitioner registration photograph endorsement form for this purpose (link opens in new window).

You'll need to submit your practitioner registration photo endorsement application form by post to Licensing Services, including your application reference number - which will be confirmed when you submit your application.
