You must obtain the consent of the parents or carers of the young person, or the consent of the young person, before you can make a professional referral to York SENDIASS.
You'll need to provide:
- your own contact details
- the details of the person who provided consent
- the young person's details
- contact details for the representative of the young person, or the young person
- details of the reason for your referral
Required information in respect of the young person includes:
- young person's name and date of birth
- young person's address
- details of young person's educational setting
Please be aware that our system will 'time out' after 10 minutes spent on each page. We suggest you gather the required information ahead of completing the form, as it's not possible to save a partially completed form, nor to retrieve information if the time out period is reached.
