If you live in York and and want to make a health or lifestyle change please complete the referral form to book an appointment with a Health Trainer, who will help provide free confidential one-to-one support and guidance, face-to-face or remotely.

Healthy Families is a free service from York Health Trainers, designed to help families with children build healthy habits, feel more confident, and enjoy a happier everyday life.

To complete this form, you'll need to provide

  • your contact information including a phone number and/or email address
  • information about any long-term health conditions, accessibility needs or disability
  • any other information which might help your Health Trainer to provide the best support and guidance to meet your needs

Self referrals

If you've been referred to the service by a health professional, such as a doctor, health worker or GP, you'll also need to provide your referrer's contact information.

Professional referrals

If you're a health professional completing a referral for an individual or family, you'll need to provide your own information as well as the details of the subject(s) of the referral.
