Use this form to make an Enhanced Resource Provision Referral for St Oswald's School.

You will need to supply the following documentation and information in support of this referral:

• CAMHS report, if applicable
• SaLT report, if applicable
• example of Independent Writing, if applicable
• recently reviewed MSP (or equivalent), if applicable
• any supporting medical reports or letters
• outline statement of description of need
• outline statement of current provision
• outline statement of evidence of interventions being used and impact - detailing what is working well
• details of teacher/TA support time allocated
• details of attendance, exclusion and attaintment levels

If the parent and child are present with you when completing this form, then you will need to provide:

• prepared statement in support of referral from the parent
• prepared statement in support of referral from the child - 'Child's Voice'

If the parent and child are not present with you, these can be provided separately at a later date.
