Leaving care

The Pathway team helps young people who have left care or are about to leave care.

Needs assessment

We (personal advisors from the Pathway team) arrange to meet you, the people who care for you and any other people who know you well.

We look at:

  • what support you need
  • what practical skills you have and what you need help
  • your health
  • your leisure interests
  • your finances
  • your accomodation
  • your education, training and/or employment

Pathway plan

During your needs assessment we write your Pathway plan. This plan says what you might need help with who will provide the help.

From then on we look at your Pathway plan together every six months to see what has changed and agree any changes that may be needed.

Once you have left care

We run groups, publish newsletters, and organise a wide range of social activities that you can take part in. Your personal advisor can tell you more about these services.

If you are new to the area

If you are have been looked after in a different area you can contact the Pathway team directly by contacting the customer advice centre or email:pathway@york.gov.uk

If you are already looked after by us your social worker can put you in touch with the Pathway team.
