You can download the application form from the right of this page. All applications must be made ten school days before the licence is required to enable us time to process the application.

A leaflet on performance licences for children is available to download on this page.

Chaperone

A chaperone acts in loco parentis (in place of a parent/carer) to the child/ren under their care when they are modelling, performing on stage, taking part in a film production, radio or television programme. A chaperone is expected to exercise the same care and control as a good parent would with a view to securing a child's health, comfort, kind treatment and moral view.

A chaperone must be registered with the LA, and undergo a compulsory Criminal Records Bureau (CRB) check, which will incur a charge. Please email or contact us directly for further information.  An application form is available on this page.
