Application forms

All forms are available to download on the right hand side of this page.

• Download the application form for a premises licence form.
• Download the notification of an interest in premises form to register a notification of interest in a premises which is the subject of a premises licence.
• Download the application form for a provisional statement to apply for a provisional premises licence. This is where premises are being built (or altered/extended).
• Download the consent of individual to being specified as DPS form if you want to give consent to being specificied as a designated premises supervisor.

What to send with your application forms

(Section 17 of Act) You must send the following with your application form:

• the appropriate fee
• the operating schedule
• a premises plan
• if the sale of alcohol is to be included, a consent form - DPS

We also request that applications are accompanied by a copy of the latest National Non-Domestic Rate Demand Notice for the premises. This is not a legal requirement, but receiving the document would assist us in processing applications.
